The file app integration in iPadOS only allows users to select a file but not a folder to open in infuse so BDMV folders cannot be played directly from an external drive. Is there any solution to this?

Unfortunately not.

Due to the complex nature of these files, in order to play them they must first be transferred to Infuse, or streamed from a network source (SMB, NFS, Dropbox, Google Drive, etc...).

Infuse 6 for iOS and Apple TV is now available!

This may be possible as I saw other apps that can get permission to access the whole drive rather than a single file via iOS file app integration.
